I am looking into becoming an A1 Assessor in health and social care. I have from my Assessor been given a workbook to read and complete task within the book. Would it be possible for an example of the following just to get me on the right track, many thanks in advance

Process - Action Planning - Name of form to record this process?
When within the assessment process is it used?
What happens to the form once it is completed?
The role of the assessor within this process?

Any help would be most appreciated.
