i dont know if its just me but i cant seem to get my head around theses PCs. just need them explained in plain english

(guessing this is about passing on information about an individuals needs via care plan, handovers, team meetings
you reflect on and challenge:
your own assumptions, behaviour and ways of working
the assumptions of others, their behaviour and ways of working
procedures, practices and information that are discriminatory
alot i know but i am really struck. just need a nudge in the right direction.
i have to complete this unit in the next few days
