Describe how you do the following and how i do this at work

support individuals to express their needs and preferences

Support individual's to understand and take rsponsibility for promoting their own health care

support individuals to identitfy how their care needs should be met

support indiviuduals to assess and manage risks to their health and well being

You need to explain how you do each of these things at work - try and think of examples from your work practice - how have you supported a client to express their preferences? etc
