1) How individuals are supported to lead the assessment and planning process and how the documentation is designed and used to maximise the individuals ownership and control.

2) How care workers support individuals to challenge unhelpful information, overcome barriers to accessing services and facilities and ensure when using them that individuals rights and preferences are promoted.

Any help appreciated

depends on your individuals that you are supporting.

mr bloggs is wanting to attend a assessment on his hearing, is he able to verbally put forward his concerns etc, are you hte "middle guy" and can pass the info over? the planning process would be getting the referal to the gp, is he able to do this himself and with how much of your support?
so think to your own work and a incident, then that will help towards the documentation (paperwork) etc. the assessment can be anything that the individual gets assessed on, ie aromatherapy...assessing to see if can affect personal medications, hydrotherapy, wanting to join but need the all clear from the doctor, etc)

at the assessemnt the consultant tells mr bloggs just to ignore the tinnitus in his ears. (why should he? there is help for that) you think this is unhelpful so how would you support mr bloggs to stand up and get proper info? all gthat should involve overcome barriers etc.
