Hello,
This needs to be written using examples from your own experience. You need to say how you support individuals to choose what services and facilities they want to access, how you find out about them and supply the information.
So you could describe how you looked at Care Plan, read that Mrs. P liked to read, you then found out about libraries, opening times, location, maybe there is a mobile one, then describe how you explained about this this and about any other service or facility she might be interested in that meets her identified needs and how you supported her to make choices.
