These aren't questins, these are performance requirements and you will need to show your asessor how you do them. So, your assessor will need to see you checking care Plan if there has been a change in how a servicee user is monitored, or confirming change with colleague/senior and assesor will need t see you actually using the changed monitoring process, maybe someone needs to be turned and this recorded every 2 hours instead of every 4 for instance, you need to be seen doing this.
