Explain the importance of recording significant conversation during end of life care?

That is probably about respecting wishes of a person and end of life. They may have requests for after they die. Recording will be in notes etc. Vey important especially if a will or wishes may be challenged. If several people have a conversation with a person at end of life and they say each time they want a cremation, for example. As the conversation has been recorded and done so several times any challenge on this by family members will probably be futile unless the can prove the person at end of life lacked Mental capacity.

The above might cover some of the legal and ethical issues you asked about in your other post.
