2.
Describe how a person centred approach enables individuals with dementia to be involved in their own care and support.

please help gotta hand this unit in tomo . . . .if i write this am i on the right lines . . . .

Each person with dementia living in a care home should have his or her own individual care plan. The care plan should summarise how staff can encourage and maintain the unique strengths of the person with dementia while meeting his or her needs for support. This plan should be reviewed at regular intervals.

Hello,
Well that's ok but you haven't said how the person with dementia is involved in the process. You need to describe how this happens in your own workplace.
