Firstly explain to them you will have to find out, then go to your supervisor manager and find out moe information regarding communicating with the service user, is there info in the care plan? if its a communication problem thats developing now due to some deterioration of there condition then report this, an assessment can take place to determine what is needed to be out into place to combat the problem.

what is the communication concern?
