Need help please!

Mildred is due to have her blood pressure checked, following the reading you discover she is extremely hypertensive and agitated.
Answer the following questions.

1-What documentation would you check prior to commencing the task?

2-What action would you take following Mildred's reading and who would you report to?

3-What other health care professionals may nedd to be informed?

4-Identify two factors why Mildred's blood pressure needed to be checked

Thank you.

1) you would have checked her observation chart to see what her last set of observtion were and medication chart in case of a change in her mediction and her care plan

2) Reasure Mildred, maybe get her to lay down on her bed you would report the reading straight away to the Nurse is charge ( staff nurse, junior sister, sister, manager)

3) GP, Houseman, registra.

4) Risk of stroke , heart attack

Hope this helps
