what information do individuals have to be able to make comments on their care. would this be that sheet that they fill in from careplan

Hi dianefoley,

Yes you advise them to read the care plan carefully an use the sheet to make any comments they want to make. You also advise them that they have the right to make comments and let their preferences be known as the care plan is being developed.
