please can you tell me how social services gps and emergency services are involved in safeguarding individuals

The assessment tool/current review cycle may no longer be appropriate due to the clients health,mobilty. For example the client may now be bed bound so the assessment may have been done when client was mobile , so new assessment will need to be done as client now at a higher risk of skin breakdown.
