describe how to support an individual to make informed choices and describe how to support an individual to question or challenge decisions concerning them that are made by others?
all i can think of is to explain to the individual that it is his or her life and that they have a right to make their own decisions. would i be right with this answer?

Hi Diane. You need to make sure that they have all the information, and understand it, in order to make an informed decision. They have the right to make their own choices and decisions under the Human Rights Act, as long as the decisions they make do not put them at risk of harm or injury and they have the mental capacity to make them. You could support them to question or challenge by speaking on their behalf but you would need to know exactly what their wishes are, and record it for them to sign.
Hope that helps
