Hi

Could anyone help with questions that I should use in a feedback form?

I was thinking of using a scale system for feedback 1 being poor 6 being excellent as I have to form a graph of my findings afterwards and thought this would be easier, is this the right way?

I am currently working on my self development work book and am finding this the most difficult so far.

Thank you in advance for your help!

You could include things like 'works well as team member' 'works well on own initiative' 'good/poor time keeping' ' communication' 'health and safety' 'equality and diversity' 'training' Think of supervisions and the topics you talk about.
I think we all find it hard to find positive things about ourselves so good luck lol.

Do you have any idea how many questions there should be? I have 7 is that enough? I just really don't want to have to do this one again after I have handed it in lol xxx
