I have stated that if you become aware of unsafe practice that one should report it to their manager / supervisor, however is there anyone else that one should report to and I also stated it within their policies ans procedures. Am I sort of one the right track

That's right and if you don't get a satisfactory outcome you could report it to the CQC or HSE or even your Union if you have one.
