Hi, some suggestions:
Advocacy is a process of supporting and enabling people to:

* express their views and concerns
* access information and services
* defend and promote their rights and responsibilities
* explore choices and options

The Mental Capacity Act 2005 provides protection and support for people who lack capacity to make their own decisions, this is done through three main organisations:
•the Court of Protection
•the Public Guardian
•the Independent Mental Capacity Advocate

Advance Care Planning, (describe what this & who can help with this)

Spiritual well-being is an integral part of mental, emotional and physical health, this doesn't change for a person that is nearing the end of their life.
Sprituality & religion are not necesarily the same thing & it means different things to different people, in a general sense it's what gives meaning, purpose & value to our lives & what makes our thoughts & beliefs personal to us.
For an individual to have their spiritual needs met can mean they have the opportunity to reflect, resolve issues & find inner peace.
