I need your help please. I have just started NVQ 4 in Health and Social Care (Adults). My first assignment from my Assessor is: Write a Reflective Account to show that you provide active support to individuals, key people and other people within and outside your organisation to enable them to identify what needs to be in place for effective communication. I would really appreciate your guidance.

Hi Ethel, you need to write about the care plans you use at work and how these are developed to identify, assess and implement the needs of the people you care for. Show how you include people, relatives, staff and other professionals in the process and respect peoples rights. also how do you access any specific aids to communication?
