explain why it is important to complete records accuratley with detail in the correct format so they are accessible and readable for those who need to access them I KNOW THAT BIT WHY! could sumone explain the problems that may arise if this is not done???? pls

if records are not lrgible and accurate it is a form of abuse

It could lead to incorrect implementation and the decision making process will be influenced adversely. Mistakes will happen e.g. medication records.
