Hi KMILES,
You need a theory for this, for example Maslow's 5 stage model (try
www.businessballs.com and search for Maslow):
I assume that you work with the elderly. Consider a man who has lived a busy life with responsibility at work and a family to care for - he reached at least stage 4. On retirement he found new interests but as his children moved away and his wife died he began to feel less needed. He came into care because he could no longer manage at home. Although he is fed and kept safe he has lost his self-esteem - down to stage 2. No one asks for his advice, he does very little all day so has nothing to talk about and no one to listen, his eyesight and hearing is failing. He has lost communication skills.
To achieve Maslows higher levels again he needs your support.
Hope that helps
Aunty Sue
